To ensure your registration, please carefully read and complete all steps listed below. IMPORTANT: You are only registered once ALL steps are completed . All FALL HIGH SCHOOL SPORTS: Physical Forms and Athletic Registration are due on or before Thursday, July 25th! NOW OPEN! I n-House Physicals will be offered on Wednesday, July 10th and Wednesday, July 17th for all high school and middle school athletes from 9:00 am-12:00 pm in the high school nurse’s office. They are open to students in grades 6-12 by appointment only and are $25 a child. (Checks should be made out to HVRSD) Please use this link to sign up for an appointment. Please note, physicals are good for 1 year (365 Days) from the exact date of the last physical. NJ Law requires that a paper copy be kept on file in the school’s nurse’s office. Download the Preparticipation Physical Evaluation Forms here - Parents/Guardians complete pages 1 & 2(if applicable). Your physician is required to complete pages 3 & 4 including the vision section. Please ensure all required signatures are provided. Note: If there is a current physical on file with the school nurse, you are only required to complete the Health History Update Questionnaire Form Please ensure all required signatures are provided. Once the Preparticipation Physical Evaluation Forms or the Health History Update Questionnaire Form is completed, please submit to the school nurses office. For your convenience and so that there is a record of the physical on this database, you can also scan it and submit completed Physical Forms online through your family account. *Important* If your child has tested positive for COVID after April 1st, 2022 and they HAVE NOT completed the POST-COVID Return to Play Protocol , they will need to have the HVRSD COVID-19 Return to Play Form filled out and signed by their physician. This can be done either in person or via Telemed. Step 2: Central High School Registration Form - This must be completed each season in order to ensure registration for each sport. Once you have completed these steps and handed in your medical forms, your coaches will be informed if you have been cleared to participate in your sport.
